Health Statement
I do not have any injuries or conditions that would prevent me from receiving an
automated chair massage (zero-gravity), nor have I been told by a health provider that I should not receive massage therapy.
I understand that an automated massage is for the purpose of stress reduction, relief
from muscular tension, general relaxation improvement, and improvement of circulation and energy flow. It is not a substitute for medical care and qualified medical diagnosis.
I understand that it is normal to feel slight soreness after a massage. The techniques are used to carry blood and nutrients to your muscles while eliminating toxins. After stimulating muscles you don’t normally use, you might experience delayed onset of muscle soreness. This is a physical response to the inflammation as your body heals
I hereby waive and release any and all rights and claims for damages that may hereafter accrue to me or which I have against Wine Down Spa, LLC for any and all injury, pain, and suffering during the chair massage, steam sauna, aromatherapy & reflexology sessions.
